To be eligible to play hockey for CYHA you must abide by MN Hockey guidelines and reside within the boundaries of ISD#12.  Open Enrollment to ISD#12 does not change residency rules per MN Hockey.  Please read about Player Eligibility below:

In order to guarantee that you will be included on our roster for the 2008 - 09 Season - Registration Forms and Payment should be turned in no later than Monday, Sept. 15, 2008.

To determine the correct playing level (initiation, squirt, 10U girls etc), click on the 2008-09 Age Chart below.  Please NOTE - players with July or later birthdays are eligible to follow their grade and register to play at the next level.  This will be verified.

2008-09 Fee Schedule 

The following forms are REQUIRED to register for hockey:  (download and complete all forms to register)

• USA Hockey On-Line Registration verification *** NEW for 2008-09 *** - Click HERE for Step by Step guide. 

You must register your child with USA Hockey and pay the $30 USA Hockey fee and $10 MN Hockey fee on-line prior to in-person registration.  Print off and bring with you, your bar-coded confirmation page.  Any child 6 or under (born January 1st, 2002 or later) - DOES NOT PAY EITHER FEE - but must still register with USA Hockey prior to in-person registration.

Your Confirmation receipt also includes the USA Hockey Waiver of Liability (which you agreed to while registering on-line)

• 08-09 CYHA Consent to Treat Form/Medical History
• COPY of  players govt issued birth certificate  ** If player skated with CYHA last season contact the Registration Director to verify if BC is on file. 
• 08-09 Volunteer Form

• Insurance information to complete forms.
• Checkbook for fee payment and volunteer deposit. (Seperate checks are needed for fee payment and volunteer deposit)

** CYHA must have a copy of all players birth certificates.  Parents will need to provide birth certificates.

Hospital or baptismal certificates are NOT acceptable.
