We would like to create an online coaching application form, similar to our board application.

Please provide me a quote to implement.

A document containing what we need to have included on this form is attached.

We have started a new feature for online forms that can be configured by you.  We have already created the first one for a season evaluation form.  We will be creating the others over the next few weeks and will release it in the next hockey upgrade that will start in late April to May.  If that timing works, then there is no charge.  If you'd like us to create something sooner, then we could create this form for $75 to $120.

Excellent Job!

3 things:

Spelling error:  Decribe your coaching strengths  (Describe)

Under:  Level Applying For:   Can you make it possible for them to select more than 1.  We need to see all that are selected.

Relation to Applicant:  Can you make the field  able to expand like the fields above so that the applicant can expand on the relationship?

The spelling error is fixed and the relationship has been changed to a text area.

For the level applying for, we have to do one of two things.  We either turn the entire list into a set of check boxes or radio buttons and the user selects as many as he/she wishes, or we give the user a specific numer of selections.  To better understand the specific number of selections, they might be able to select exactly 3 or 4 levels.  Do you have a preference?

you are needs a space in above description.

Please add an email space for each reference.

I think a toggle box next to each level will work great for the levels.
