Expense Reports

Centennial Youth Hockey Association (CYHA) will reimburse approved expenses incurred on behalf of the association.

How to Submit an Expense

To request reimbursement:

1. Complete the CYHA Expense Reimbursement Form.
2. Include a copy of all applicable receipts or proof of payment.
3. Provide enough information to identify the purpose of the expense, including the team, program or CYHA activity associated with the purchase.
4. Submit the completed reimbursement request and supporting documentation to your level director for processing.

For example, if your player is a Squirt, Peewee, and Bantam send your reimbursement request to the Boys Director. If your player is a U8, U10, U12, U15, and U19, send your reimbursement to the Girls Director. If your player is a Mite, send your reimbursement to the Mite Director.

Goalie Equipment Reimbursements

Goalie equipment reimbursements are subject to the requirements and reimbursement limits established in the CYHA Goalie Discount and Reimbursement Program.

Goalie reimbursement requests should be submitted to the CYHA Goalie Director for review. Final approval is made by the CYHA President.

Receipts are required for reimbursement.

Coach & Volunteer Reimbursements

CYHA reimburses certain required expenses for eligible coaches, student coaches and volunteers. These may include approved USA Hockey registration fees, coaching clinics and required age-specific modules.

Please complete all required certifications and submit your receipt along with the CYHA Expense Reimbursement Form.
